Direct Primary Care Program $149 one-time fee. Followed by a $50 monthly membership.

Great for clients without an established primary care provider (PCP) and seeking to establish care at Face It By Dr. G.

24/hr access to your provider  

 

Does not include:

Vaccines and immunizations (we do not provide these services)

Narcotic medications (we do not provide or dispense these medications)

Great for clients seeking a wellness physical (no acute illness during the time of the visit), and do not currently have an established primary care provider (PCP).

Evaluation of acute conditions

Vaccines and immunizations (we do not provide these services)

Follow up visits 

Great for clients with acute illnesses. Avoid the emergency room (ER) and urgent care wait times for non-life-threatening emergencies. This service is not to be used for chest pain, severe headaches,  abdominal pain, or shortness of breath.

Great for: clients seeking management of a chronic disease ex: Hypertension (high blood pressure), Asthma, Hyperlipidemia (high cholesterol), Diabetes, who have an established primary care providers (PCP) but need routine refills under various circumstances (ex. provider unavailable)

Physical exam

90-day refill supply of a current medication(s) at the current dose

 

Does not include:

New prescription orders (initiation of a new medication)

Increasing the dose of current prescription(s) orders
