Service Description

Direct Primary Care Program $149 one-time payment. Followed by $50 monthly membership. Great for: clients without an established primary care provider (PCP) and seeking to establish care at Face It By Dr. G. Includes: Annual Physical exam Completion of forms Prescription medications Labs orders (including titers) 24/hr access to your provider Urgent/Sick visits at no extra cost Does not include: Vaccines and immunizations (we do not provide these services) Narcotic medications (we do not provide dispense these medications)

Cancellation Policy

All appointments canceled within 24 hours are fully refunded, less the $25 booking fee. For IV hydration, appointments canceled within 6 hours incur a non-refundable $150 fee, and appointments canceled within 2 hours are 100% non-refundable. For package purchases, unused portions are not refundable. Lastly, for all services, if a nurse has already been dispatched to the client’s location or if you are not present at the given address within 15 minutes of the provider's arrival, this will be considered a no-show, and you will be charged 100% of the non-refundable fee; for services provided in a package, you will forfeit a session from the scheduled package.
